How to refer a patient
For GPs, case managers, support coordinators and self-referrers. Online submission is temporarily switched off, so please refer by phone, email or the printable form below — we'll call your patient within one business day.

We've switched off online submission while secure referral handling is being finalised. We don't want you to type patient details into a form that can't accept them, so please use one of these instead — all three reach our intake team the same day.
