How to access the funding or begin treatment

  1. 1Report your injury to your employer and obtain a certificate of capacity from your nominated treating doctor.
  2. 2Lodge or confirm your workers compensation claim with the insurer (icare or the self-insurer).
  3. 3Contact Blacktown Physioclinic, or ask your GP or case manager to send us a referral.
  4. 4Provide your claim number and insurer/case manager contact so we can confirm approval for the requested service.
  5. 5Attend your initial assessment — we will submit any Allied Health Treatment Request (AHTR) the insurer needs before ongoing sessions.
