Referral requirements
What each funding stream needs
Here's what each pathway typically needs — we'll follow up with your patient for anything missing.
Workers Compensation (SIRA)
- Claim number and insurer name
- Current Certificate of Capacity
- Nominated Treating Doctor details
- Approved treatment request (if in place)
CTP Motor Vehicle
- Claim number and CTP insurer
- Date of accident and injuries claimed
- GP referral or medical certificate
- Any relevant imaging reports
NDIS
- Participant name, NDIS number and DOB
- Plan management type (self / plan / NDIA)
- Plan manager or support coordinator contact
- Relevant goals from current plan
Support at Home (from 1 Nov 2025)
- My Aged Care referral code
- Approved classification level and service type
- Provider / care partner contact if assigned
- Any allied-health goals in the support plan
Medicare GPCCMP
- GPCCMP referral form signed by the GP
- Requested discipline(s) and number of sessions
- Relevant diagnoses and management goals
- Patient's Medicare number (via secure upload only)
DVA
- D904 referral from the GP
- Gold or White Card number
- Accepted conditions (White Card)
- Any home-visit access requirements
Privacy & security
Referrals travel over TLS and staff notification emails contain no patient information. Records are handled under the Australian Privacy Principles and the NSW HRIP Act. Online attachments aren't available yet — email documents to the clinic or call reception to arrange a secure transfer.

- Patient full name and one contact method
- Requested discipline (or "not sure — please advise")
- Funding stream (or "not sure")
- Optional: brief clinical notes
