The GP Chronic Conditions Management Plan (GPCCMP) replaced the older GP Management Plan and Team Care Arrangement from 1 July 2025. Eligible patients access up to five subsidised allied-health visits per calendar year.
How it works
- Your GP prepares a GPCCMP and issues a referral to eligible providers
- Up to 5 subsidised visits per calendar year across all allied-health disciplines combined
- Medicare pays a rebate; a small out-of-pocket gap usually applies
- You can use visits across physiotherapy, exercise physiology and other allied-health providers

Your GPCCMP referral, your Medicare card, and any relevant scans or reports.
