NDIS Allied Health Services in Blacktown

We work with NDIS participants of all ages to build capacity, participate more fully and pursue the goals in their plan. Our team offers physiotherapy, exercise physiology, occupational therapy and specialised assessments — in our accessible Blacktown clinics or, where appropriate, through home visits.

Every participant is different. We take a person-centred approach that starts with your goals, priorities and preferences, and works with the people and supports already in your life.

Services available under this pathway

Under NDIS therapy supports, we may deliver one or more of the following where they align with the participant's plan.

NDIS Physiotherapy

Movement, strength, mobility and pain management to support daily activities and independence.

NDIS Exercise Physiology

Clinical exercise for disability-related health, function and community participation.

NDIS Occupational Therapy

Practical support for daily living, home and community activities, and environmental strategies.

Functional Capacity Assessment

Structured assessment to help describe function for planning and review purposes.

Assistive Technology Assessment

Assessment and reports for mobility, seating and daily-living AT within our scope.

Home Visits

Selected supports delivered in the home when clinically appropriate.

Funding-management options

Self-managed

Participants who manage their own funding can book services directly and pay from their plan.

Plan-managed

A plan manager pays our invoices from the participant's plan on the participant's behalf.

NDIA-managed

NDIA-managed participants are accepted only where Blacktown Physioclinic's current NDIS registration covers the requested support. Please contact us to confirm before booking.

How to access the funding or begin treatment

  1. 1Confirm the participant has current NDIS therapy support funding relevant to the request.
  2. 2Contact us or submit a referral with the participant's goals and any supporting information.
  3. 3We will confirm how the participant's funding is managed and what we can offer.
  4. 4Complete an initial assessment to agree goals, session frequency and any reports required.
  5. 5Service agreement is confirmed with the participant, family or nominated representative.

What information to send us

  • Participant's name, date of birth and NDIS number
  • Plan start and end dates
  • How the plan is managed (self, plan or NDIA managed) and plan manager contact if relevant
  • Goals the participant would like therapy to support
  • Relevant prior reports, imaging or specialist letters
  • Whether home visits are being requested
