Allied Health Under Support at Home

Support at Home is the Australian Government's aged-care program helping older people live independently at home for longer. Our Blacktown team provides physiotherapy and exercise physiology that helps eligible participants stay mobile, confident and connected — in our accessible clinics or in their own home.

How we can help

We focus on the things that keep older people well and independent — moving safely, staying strong, and recovering after illness or hospital stays.

Services available under this pathway

We work with participants, families, care partners and Support at Home providers to deliver allied health services approved in the participant's care plan.

Physiotherapy

Assessment and treatment for mobility, pain, post-operative recovery and reconditioning.

Exercise Physiology

Clinical exercise for strength, balance, chronic conditions and cardiovascular health.

Falls Prevention

Evidence-informed balance, strength and confidence programs for older adults.

Post-hospital rehabilitation

Structured recovery after illness, hospital admission or surgery.

Home visits

Selected services delivered in the participant's home where clinically appropriate.

Clinic-based rehabilitation

Delivered in our accessible private rooms and equipped rehab gym.

For older people, families and carers

We use plain language, take time to listen, and design programs around what matters to the participant — walking to the shops, getting up from a chair, playing with grandchildren, staying in their own home. Sessions can happen in our clinic or, where suitable, in the home.

For care partners and Support at Home providers

We work alongside your care partners with clear communication, written progress reports and responsive scheduling. Referrals are simple, and we confirm approved services and fees before commencing care.

How to access Support at Home

  1. 1Register with My Aged Care.
  2. 2Complete an aged-care needs assessment.
  3. 3Receive approval and allocated funding under Support at Home.
  4. 4Choose a Support at Home provider to coordinate your care.
  5. 5Agree on approved allied-health services in your care plan.
  6. 6Submit a referral to Blacktown Physioclinic — we will confirm approved services and scheduling.

What information to send us

  • Participant's name, date of birth and My Aged Care reference
  • Support at Home provider name and care partner contact
  • Copy of the approved care plan and the services requested
  • Any GP referral, hospital discharge summary or specialist letters
  • Whether home visits are being requested and the participant's address
