Restorative Care Pathway Allied Health Services

The Restorative Care Pathway is a short-term, goal-focused pathway within Support at Home for eligible older people. Our multidisciplinary team supports participants to rebuild strength, mobility and confidence so they can keep doing the things that matter to them.

How we can help

Restorative care works best when it is practical, personal and progressive. Our team designs a plan around meaningful goals — a safer transfer, a longer walk, cooking a meal, returning to a hobby — and reviews progress regularly.

  • Examples of difficulties that may benefit from restorative care:
  • Reduced strength or mobility
  • Deconditioning after illness or hospitalisation
  • Falls or increased falls risk
  • Reduced confidence walking
  • Difficulty with transfers
  • Reduced independence after surgery
  • Difficulty completing everyday activities

Services available under this pathway

Where approved in the goal plan, participants may access a combination of the following.

Physiotherapy

Assessment and treatment for mobility, pain, post-operative recovery and reconditioning.

Exercise Physiology

Structured clinical exercise to rebuild strength, balance and endurance.

Occupational Therapy

Practical strategies and equipment to support everyday activities.

Falls Prevention

Balance, strength and confidence programs tailored to the individual.

Post-hospital rehabilitation

Focused recovery after an acute admission or medical illness.

Post-operative rehabilitation

Staged rehab following surgery, in line with surgical protocols.

Mobility and transfer training

Safe standing, walking, and getting in and out of chairs and beds.

Daily-living assessment

Assessment of everyday tasks to identify practical improvements.

Assistive Technology Assessment

Assessment for aids and equipment within our scope.

Home-based rehabilitation

Selected services delivered in the home where clinically appropriate.

How to access the Restorative Care Pathway

  1. 1Aged-care assessment or Support Plan Review confirms current needs.
  2. 2Eligibility for the Restorative Care Pathway is decided through the aged-care system.
  3. 3A goal plan is prepared with a restorative care partner.
  4. 4Approved multidisciplinary services commence — this is where we can help.
  5. 5Progress and outcomes are reviewed during the pathway.
  6. 6An exit plan and ongoing recommendations are prepared toward the end of the 16 weeks.

What information to send us

  • Participant's name, date of birth and My Aged Care reference
  • Restorative care partner and Support at Home provider contact
  • Approved goal plan and services requested
  • Recent hospital discharge summaries, GP or specialist letters
  • Whether home visits are being requested and the participant's address
