The Restorative Care Pathway is a time-limited, goal-directed program within Support at Home for older people who can regain function with intensive allied-health input.
- Up to 16 weeks of goal-directed multidisciplinary care.
- Focused on improving or maintaining independence, not long-term maintenance.
- Requires a My Aged Care assessment and an allied-health lead.
Who it's for
Older people who've had a functional decline — often after a fall, hospital admission or illness — and are likely to benefit from intensive, time-limited allied-health input.
What's included
Multidisciplinary allied-health care (physio, EP, OT, dietetics, speech pathology as needed), equipment, home modifications and coordinated goal-setting with the participant and family.
