What an Allied Health Treatment Request (AHTR) means

The AHTR is the SIRA form allied-health providers use to request approval for treatment sessions under a workers-compensation or CTP claim.

  • It documents diagnosis, plan, session count and functional goals.
  • Insurers typically respond within 5 working days.
  • It's a request, not a bill — invoices are lodged separately once sessions are delivered.

What's on the form

The AHTR captures the presenting condition, objective findings, planned interventions, number of sessions requested and the functional or return-to-work goals those sessions will address.

The physiotherapist and the injured worker both sign the form.

How approval works

The completed AHTR is sent to the insurer's case manager. SIRA's benchmark response time is 5 working days.

If more information is needed the insurer may request a treating-team conference or a peer-review. We respond within 48 hours to keep treatment on track.
